
Ava Isabel Esposito, CRNP
Family Medicine
Ava Esposito is a board-certified Family Nurse Practitioner through the American Academy of Nurse Practitioners. She completed her Master of Science in Nursing at Thomas Jefferson University. Prior to starting her career as a nurse practitioner, she earned her Bachelor of Science in Nursing at Thomas Jefferson University and went on to work as a nurse at Jefferson Abington Hospital, caring for patients on medical/surgical, neurology, and cardiac units. In recognition of her compassionate nursing care, Ava was honored with a DAISY Award during her time at Abington Hospital.
Ava began at Chalfont Internal Medicine as a nurse practitioner student, receiving extensive clinical training under the direct supervision of Dr. Shackelford. While completing her master’s degree, she also worked as a registered nurse within the practice. Through these roles, Ava built a strong foundation and gained a deep understanding of the exceptional care patients can expect at Chalfont Internal Medicine.
Ava provides primary care to adults aged 18 and older. She values building long-term relationships with her patients and supporting their health through individualized care grounded in evidence-based medicine.
Ava grew up in Montgomery County and is a graduate of Wissahickon High School. She has remained closely connected to the community and now lives in Doylestown with her husband and their two dachshunds. Outside of work, Ava enjoys spending time with family, hiking, and practicing yoga.
